Terms and conditions of affiliation to the ICE network

I agree to accept ICE VDU Eyecare Vouchers, that are within their expiry period, in exchange for a full eye and eyesight examination and, where required solely and specifically for VDU use, a pair of basic frames and single vision spectacles.

I further agree to allow the entitled patient the alternative option of a £25 contribution to the cost of upgraded lenses and frames of their own choice.

I undertake to complete the Certificate of recommendation included in the voucher and return it to the patient at the end of their visit.

I will also allow any voucher holder to obtain a £15 discount on the sale of any spectacles and frames costing £125 or more.

I will complete the voucher details online, or return the ICE VDU Eyecare Voucher within 30 days of their expiry date, and at the end of the next calendar month, will receive the sum of £18 for each voucher returned..

I understand that I will not be paid for any vouchers received by ICE beyond 30 days from the date of expiry, and that I am responsible for the safe delivery of the vouchers.
